Once you have filled out the form, please click the Submit button.

• If paying by EFT: Bring proof of payment with you to the dog club on the next Sunday, along with your dog’s current vaccination certificate.

• If paying on Sundays: We accept cash only. Electronic payments will not be accepted.

All dogs attending classes are required to be vaccinated. If your dog is not vaccinated, you won't be able to attend classes.
